By submitting this form, I understand that participation in a dance and tumbling program involves risk of injury. I authorize Studio 55 to secure any medical treatment my child might need. I agree to indemnify and hold harmless Studio 55 and its staff from any and all claims to personal injury sustained while participating in this program.
